NHS, Healthcare & Care Worker Mortgages

NHS staff, healthcare workers and carers often have pay that does not fit a simple salary box: shift enhancements, bank work, overtime, rotations and agency or locum contracts. Lenders treat these in different ways, so knowing how your income is likely to be viewed can make a real difference to what you can borrow.

    Who this guide is for

Nurses, midwives, healthcare assistants, paramedics, doctors, allied health professionals, NHS admin and support staff, home and residential carers, and agency or bank workers across health and social care.

    Key topics covered

    • How lenders view basic pay, unsocial hours and shift enhancements
    • Using NHS bank, overtime and second jobs
    • Fixed-term contracts, rotations and resident doctor training posts
    • Locum, agency and self-employed healthcare work
    • Carers and care workers with variable hours

    Basic pay and shift enhancements

Your basic salary (for example your Agenda for Change band and pay point) is the starting point. Unsocial hours payments and shift enhancements can often be included too, but lenders differ: some use 100% if it is regular, others only part of it, and many want to see a track record on your payslips or P60.

Bank shifts, overtime and second jobs

Lots of healthcare workers top up their income with bank shifts or overtime. Some lenders will consider this if it is regular and can be evidenced, often looking at 3 to 12 months of payslips, or two years of history. A second job, including bank work with a different trust, can sometimes be used as well.

    Keep your evidence tidy

Recent payslips, your latest P60 and bank statements showing the income help lenders see that the extra pay is regular rather than one-off.

    Fixed-term contracts and rotations

Resident (junior) doctors on training rotations, and staff on fixed-term or secondment contracts, can still get a mortgage. Some lenders are comfortable with rotational and fixed-term contracts in healthcare, particularly where there is a history of renewals or a clear training pathway.

    Locum, agency and self-employed work

Locum doctors, agency nurses and self-employed carers may be assessed on day rates, contract income or accounts and tax returns depending on how they are paid. The right lender depends on how long you have worked this way and how your income is evidenced.

    Carers and care workers

Home carers, residential care workers and live-in carers often work variable or zero-hours patterns. Some lenders will average your income over a period of time. If you receive Carer's Allowance or other benefits, some lenders can take this into account too — see our benefits guide for more.

    Schemes and other support

Some local and shared ownership schemes give priority to key workers, including NHS and care staff. Availability varies by area and changes over time, so we check what is open to you when we look at your options.

    Things to consider

Every lender has its own criteria, and borrowing amounts are not set until a lender has assessed your full application. Maternity leave, reduced hours, or a recent change of trust or employer can all affect how your income is assessed, so it is worth talking these through early.
